Youth Programs
Adolescence and young adulthood can be a challenging phase, especially for youth in the State’s foster care system who face unique obstacles during this transition. Our youth programs focus on providing comprehensive support tailored to their needs, including mental health services, life-skills development, and mentorship opportunities. By addressing both the immediate and long-term challenges these young people encounter, we aim to empower them with the tools, resources, and relationships necessary to thrive as they enter adulthood.
Pathways & Passages Youth Program
Our Pathways and Passages Youth Program is a comprehensive initiative designed to support youth aged 16-23 transitioning out of foster care. Recognizing the adverse childhood experiences (ACEs) and trauma these youth have faced, the program is based on a trauma-informed, youth-centered approach to provide holistic support and empower youth in their journey towards independence and success. The program is built on the principles of safety, trustworthiness, peer support, collaboration, and empowerment, ensuring that youth have a say in decisions affecting their lives, and their strengths and cultural backgrounds are respected. Trauma-Informed Case Management and Individualized Planning form the foundation of the program, where each youth undergoes a thorough initial assessment to understand their strengths, needs, goals, and trauma history. Personalized transition plans are developed, incorporating trauma-informed care principles and ensuring that youth feel heard and supported through regular weekly check-ins with care managers.
Peer Coaching & Support
Our Peer Coaching and Support Program incorporates peer leaders—young adults from diverse backgrounds who have personal experience with behavioral health issues, foster care, or similar circumstances. These peer leaders provide mentorship, relatable guidance, and support to the youth in our program, enhancing the overall support system with their unique perspectives.
